Financial Commitment to Our Patients

Family Health West is committed to keeping healthcare costs as low as possible and to creating a financial payment plan that meets patient resources.

We know that paying for medical care can be difficult for many families. As part of Family Health West’s commitment to our patients, we provide counseling and assistance to help individuals apply for available programs that may help cover unpaid hospital bills.

Patient may qualify for the following types of help:

  • Colorado Medicaid application assistance
  • Colorado Indigent Care Program (CICP) application assistance
  • Payment plans that allow for monthly payments over time
  • Discounts for self-pat patients at the time of service
  • A Family Health West Program for patients who do not quality for any type of government Assistance program
  • Question:How do I apply for financial assistance?

    Answer: Complete our Application for Financial Assistance and schedule an appointment with our Patient Financial Counselor at 970-858-2142. The Application is available from our Registration Department or the Financial Counselor. It is also available below.

    Question: How does Family Health West determine eligibility?

    Answer: We will review the details of your application which includes current income, expenses, and family size. You will be officially notified of our decision in writing within 30 days of receipt of the completed application.
