Our Continuum of Care ensures that our patients remain with their family at Family Health West as their health care needs change. The patient feels secure in knowing that their needs can be met without the undue stress and anxiety of moving to an unfamiliar place. We have on-site nursing and social work assessments to assure appropriate, efficient placement and treatment. Coordinated physical rehabilitative programs allow the patient to recover as quickly as possible, close to home.
