Continuum Provides Continuity of Care and Quality of Life.
Our Continuum includes facilities that offer
a variety of Health Care Services:

A 10-bed HOSPITAL that includes
on-site Physical Rehabilitation Therapy Services
to include Physical,
Speech and Occupational Therapies; Medicare Skilled Nursing
Services (Swing Beds); and on-site Lab, X-Ray, and
Emergency Room Services that are also available to individuals
of all ages in the
community on an outpatient basis.

PHYSICAL
REHABILITATION
THERAPY CLINIC providing
Physical, Speech and
Occupational Therapies. Services are offered to inpatients and
to individuals of all ages in the community on an outpatient
basis.

A 120-bed ADULT EXTENDED LONG-TERM CARE
NURSING HOME that includes Specialized Alzheimer's Care
"The Main Street Experience," a Progressive, Innovative,
Award Winning Program offering each
resident Quality of Life and Dignity.

A 26-bed SECURE ASSISTED LIVING
residence, The Willows, an elegant personal setting designed
for individuals who have Alzheimer's, Dementia or Confusion.

A 90-bed ASSISTED LIVING residence,
The Oaks, offering Country Charm and Personalized Care
in a nurturing family environment.

RESPITE CARE
is offered in our Assisted Living and Adult Extended Long-Term Care Nursing Home
environments.

PSYCHOLOGICAL
SERVICES are provided by an experienced Clinical
Geropsychologist and a Nurse Psychotherapist with a master's degree
in psychology. These services are an essential part of our
Family Health West Team.

- Our Continuum provides prompt access to our
Acute
Care Hospital, Physicians, Medicare Skilled Nursing, and
Physical Rehabilitation Therapy Services, as well as
on-site Emergency Room, Lab and X-Ray Services.
- On-site nursing and social work assessments
assure appropriate, efficient placement and treatment.
- Each individual receives Quality of Care
at an appropriate level for their current condition.
Functional declines are addressed through Therapies and Restorative
Nursing Programs that are designed to stem unnecessary
declines and promote Quality of Life.
- Physical Rehabilitative Programs are
tailored for individuals in order to challenge and motivate them to
maintain and enhance cognitive and motor skills, and to increase or
maintain the overall functioning of the resident / patient - thus
promoting the highest levels of Self-Sufficiency, Self-Worth, and
Dignity. This allows the resident to remain in the least
restrictive living arrangement or their present setting for as long
as possible - thus "aging them in place."
- Family Health West recognizes that the families
and loved ones of our residents also need Support, Caring and
Attention. We feel it is very important to offer support to them. We offer Family Support on a
one-to-one basis or through our Support Groups.
