DEPARTMENT JOB TITLE BRIEF DESCRIPTION
Activities Activities Specialist Assess, plan, coordinate and document activity programs to meet the individual needs and interests of our residents.
Beauty Shop Beautician Provide Beauty Shop services to our residents including hair cuts, perms, coloring,  waxing, etc.
Dietary Services Dietary Aide Participate in routine food service activities including food preparation, distribution and clean-up.
  Cook Provide a high quality, well-balanced and nutritious meal each day.
  Hospitality Aide Assemble food tray for each resident according to his/her nutritional needs.  Pass food trays and assist residents with cutting foods, pouring beverages, etc.  Pass snacks to residents and chart food and fluid intakes for each resident.  Bus tables, wash dishes, etc.  Restock nursing refrigerators with supplements.
Health Records Services    
Hospital Services Registered Nurse Assume charge of the hospital and emergency room areas.  Directly or indirectly provide all aspects of patient care in routine and emergency situations.  Make critical decisions based on assessment and experience.  Initiate effective communication with family, physicians and staff.
  Certified Nursing Assistant Assist nurses and physicians with routine and emergency care of patients.  Monitor, evaluate and address the personal needs of patients.
Environmental Services Housekeeper Provide sanitary environment for residents, patients, staff, and visitors.
  Laundry Tech Keep all linen clean and marked.  Deliver linen back to designated areas.
Long-Term Care Nursing Charge Nurse Provide and supervise total nursing activities in assigned unit during each duty, each day.  Assume responsibility for the quality of care provided to residents.  Help create a caring, family environment for residents, their families, and staff.
  Certified Nursing Assistant Work as a partner with the professional nurse and assist in meeting basic routine hygienic, nutritional, functional, social, and spiritual needs of residents.  Work with residents who have restorative or functional maintenance needs and assist residents in gaining or maintaining their highest possible, practical level of function. The Oaks - Assisted Living Resident Aide / CNA Give oversight and assistance to residents in their daily living activities.
Physical Rehabilitation Therapy Services Occupational Therapist Evaluate, plan, direct, and administer occupational therapy modalities of treatment as prescribed by a licensed physician.  Administer treatments and occupational agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability. Assist resident to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.
  Occupational Therapy Assistant Assist the Occupational Therapist in administering an occupational therapy program in hospital, related facilities or community setting for physically, developmentally, mentally or emotionally disabled clients.
  Physical Therapist Evaluate, plan, direct and administer physical therapy modalities of treatment as prescribed by a licensed physician.  Administer treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability.  Assist patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.
  Physical Therapist Assistant Direct, modify and administer physical therapy modalities of treatment as supervised by the staff Physical Therapist.  After the Physical Therapist has evaluated the patient, administer treatments and physical agents as directed.  Aid with restoration of patient functioning to prevent disability following injury, disease or physical disability.  Assist patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.
  Speech Therapist Administer direct application of speech-language pathology treatment modalities as prescribed by a referring physician.  Evaluate, plan, direct and treat all patients referred for speech language pathology treatments to restore function and prevent disability following disease or injury.  Assist patients to obtain maximum performance and function in society while learning to live a routine "normal" life within the constraints of their disability.
  Rehabilitation Aide Keep departmental records as directed by the Director of Physical Rehabilitation Therapy Services.  Provide various supportive functions necessary for operation of the department.

Social Work Social Worker Provide supportive services to residents and their families through emotional and financial counseling.  Strive for resident and  family to experience a smooth and timely transition into the Family Health West's Continuum.  Assess and provide appropriate placement for residents in the least restrictive setting.  Work with the multiple disciplines of Family Health West and the community, to uphold at all times the dignity and rights of each resident.
The Willows - Secure Assisted Living Resident Aide / CNA Provide a safe, comfortable and friendly atmosphere for residents.
The Willows Facility Nurse Assume responsibility for the quality of care provided to residents.  Help create a caring, family environment for residents, their families and staff.
