WHO WE ARE
Thank you for your interest in our organization.
Family Health West continues to be Western Colorado's Premier,
Private, Nonprofit, locally owned 501(C)(3) corporation dedicated to
providing "Excellent Health Care to our Community. Our
organization has served rural Western Colorado for over 60 years
achieving a position of "Excellence"
in Health, Alzheimer's and Elder Care. Family Health
West fills the niche of providing a Unique Innovative Continuum of
Health & Elder Care Services using an Award Winning and Nationally
recognized Social Model Approach. Our Continuum includes
Hospital, Physical Rehabilitation Therapy Clinic, Adult Extended Care /
Nursing Home and Alzheimer's Care, and two Assisted Living Residences.
We have over 275 employees and 200 volunteers serving over 245 special
residents who live on our campus, and seniors and individuals of all
ages in our community. Almost 85% of our clientele are
predominantly low-income.
Family Health West has remained on the leading edge of
rural health care delivery.
We use a
Social Model
approach to patient care. In 1997, we opened the doors to the
Main Street Experience - A Social Model for Alzheimer's and Elder Care.
This model offers a Family Environment that enhances the
Quality of Life of our patients. The Social Concept is based
on the positives, focusing on wellness, enhancing the environment
to assist each patient to maintain or improve and to prevent
unnecessary rapid decline in functioning - ultimately providing a
Higher Quality of Life and Dignity. We continue to design and
implement new State-of-the-Art Programs to provide Quality
Care to individuals of all ages and the elderly in our community.

MISSION STATEMENT
"Family Health West will provide each individual
with the most appropriate, professional care available, while
maintaining their personal Dignity and supporting the Highest
Quality of Life possible." Our Mission's intent is to provide
Quality Health Care Services to meet the growing health care needs of
our community. We offer services to individuals of all ages
in our community through our expanded Hospital, Medicare Skilled
Nursing, Physical Rehabilitation Therapies, X-Ray and Laboratory
Services, and continue to provide State-of-the-Art Senior, Elder and
Alzheimer's Care. Our Eldercare services will continue to include
the at-risk, low-income elderly. |

WHY WE NEED PHILANTHROPIC SUPPORT
Almost 85% of our clientele are low-income individuals
who are on
Medicaid or Medicare. Oftentimes, the daily rate paid by the
Medicaid / Medicare systems does not cover the costs of providing
Quality Care. Usually, only the basics of resident care are
covered and it doesn't include the full cost of providing
Quality Care, purchasing much needed equipment, upgrades in
our present patient care facilities such as painting, carpeting,
furnishing, etc., and the costs of providing Special Social Model
Programs offering Quality of Life. Funding of these vital areas presents
a challenge to us.
WHY FUND-RAISING?
Fund-raising helps cover the difference
between our costs and the reimbursement we receive, and to cover costs
of Special Services and Programs which we provide that are not
covered by the Medicaid / Medicare benefit.

It is only through the generosity of our donors that Family Health West
can acquire much needed vital equipment, remodel, upgrade or expand
our facilities, and continue to develop State-of-the-Art Programs
