History: Family Health West, located in Fruita Colorado, has remained on the
leading edge of rural health care service delivery for 60 years.
Our organization had its beginnings in the mid-1940's as a 20-bed
hospital. The hospital was a result of a community effort to
provide health care for residents of rural Mesa County in Western
Colorado. Local farmers and ranchers drove to Utah and brought
back loads of bricks and supplies to build the hospital. Although
the hospital weathered many crises in the early years, it survived
because of the community's commitment and dedication to maintaining
local health care. Family Health West is recognized as a pioneer
and Leader & Trendsetter in Health Care Services for individuals of
all ages in our community and in the Implementation of Innovative
Alzheimer's, Senior and Elder Care.

our approach to providing Quality Health Care Services for individuals of all ages in our community,
and Alzheimer's, Senior, and Elder Care.

a new model - an Expanded Family Environment where the elderly go to
Add Life to their 'Autumn Years' and to meet the Growing Health
Care Needs in our Community by Expanding our Health Care Services to
Individuals of All Ages.
Family Health West's goal is to continue to develop
and provide State-of-the-Art Programs that offer the least
restrictive social setting, which will result in a Higher Quality of
Life for our residents and patients, utilizing a, Loving-Touch
approach. Our objective is to continue to provide Physical Rehabilitation Therapy Services and Functional Maintenance Concept of Care Programs through our
Therapy, Restorative Nursing, and
Activities programs that are aimed at enabling our residents and
patients to function
at their highest possible level, on a daily basis, ultimately providing
Higher Quality of Life, Self-Worth and Dignity.
In the 1980's, Family Health West began to implement a
broader range of health care services to meet the needs of our
community. At that time, our organization developed a Unique
Continuum of Health Care which included Senior, Elder and
Alzheimer's Care Services .
Our Continuum now includes:
Physical Rehabilitation Therapy Services,
Medicare Skilled Nursing Beds, (Swing Beds)
On-site Emergency Room, Lab and X-Ray Services.
All Hospital
services are available for inpatients and to individuals of all ages in the
community on an outpatient basis.
Family Health West has over 275 employees serving 245
very special residents and patients who live on our campus. We have been the
area's premier provider of Alzheimer's and Elder Care since
the mid-1980's. Family Health West fills the niche of providing a
Unique Continuum of Health Care based on an Award Winning
Social Model for our clientele. To our knowledge
there is no duplication of this Continuum of Care in Colorado.
Our organization has remained on the leading edge of rural health care
service delivery, and has achieved a position of "Excellence" in
providing Health Care Services to our Community, including Award
Winning Care of Seniors and the Elderly.
Family Health West developed and uses a Social Model
approach to resident care. In 1997, we opened the doors to "The
Main Street Experience" - a Social Model for Alzheimer's
and Elder Care. Our model utilizes a new
concept in Alzheimer's care designed to stimulate the long-term memories
of Alzheimer's patients in a positive and pleasant manner, and to
increase the Fun part of the day. The most gratifying merit
is the wonderful difference this unique approach is having on the lives
of our individual residents.
This model offers a Family
Environment designed to enhance the Quality of Life of our
residents. The environment is complete with simulated Main
Street shops such as a general store or flower shop; additional homelike
areas such as a room with fireplace, kitchen, family dining room;
architectural enhancements incorporating large windows and raised
ceilings; and an outdoor courtyard. We have extended the Main
Street concept to the "Neighborhood" where Cottage Fronts adorn
resident rooms. Each resident's room is receiving a cottage
front complete with window, porch light, mailbox, fence, and window box
with flowers. Residents can now identify their "home" by its
unique siding, color and style.
Our Award Winning Social Model has received National Recognition and Prestigious
Awards.
Family Health West has also developed a guidebook to assist other
health care organizations develop their own Social Model or "Main Street Experience."
