Family Health West's Social Model
provides resident-focused care. The program is designed to tap
into the pleasant memories and experiences of the past and add Fun
in the day.
We support an Expanded Family Environment that individually
enhances the Quality of Life of each resident and offers an
Alternative to Loneliness with Meaningful Activities and
Pleasant
Surroundings where Stimulation and Wellness are the focal points - thus
adding Life to their "Autumn Years."
We promote the conviction that Eldercare is much more
than just custodial care. By using our Continuum of Care
and community resources we provide care that assists residents to
prevent unnecessary declines if possible, and to improve or maintain at their
highest possible level of functioning and independence.
We use a Human Approach to providing care.
Touch, a simple hug or pat on the hand, soaking the feet in warm water
followed by a foot massage, a nice walk, listening to music, enjoying
pets and children's visits, or other one-to-one attention is the human
approach being offered.
The Social Approach lends itself to the
redirection of resident behavior. By
this approach to
Eldercare, we look at and examine the connection between a
resident's health and his/her social, spiritual and psychological
experiences. The care is designed to contribute to the overall
well-being of the resident on all levels. Our
programs allow for innovation, creative ideas, stimulation, and ultimately
Quality Care designed by individual needs. Staff members focus on
building a relationship with the resident so that they can get to know
and understand the individual resident. A "Best Friends / Family Approach"
is used. Staff focus on promoting stimulation, fun and higher
functioning. Resident behaviors are redirected by using creative,
fun approaches, and utilizing the family approach. The highlight
of the day is focused on resident individual social needs and
bringing Fun into their lives. By focusing on resident individual preferences, care takes place as directed
by the resident's need, not according to staff schedule.
Caregivers provide the highest and most appropriate level of Care and
Caring by recognizing each resident as an individual with his/her
own Unique Life History and Needs.
Our goal is to provide an environment in which our residents can live
and work towards maintaining their Highest Level of Functioning
possible and enjoy Quality of Life.
